REFERRALS

This form should be completed by a professional who believes their client would benefit from help from Dartmouth Friday Hub for a short time.

• One food parcel per week
• The referral will cover six parcels.
• These do not have to be taken in consecutive weeks.
• A referral may be renewed.
• If necessary, we can deliver parcels to someone who is ill / disabled / isolating.

What to do with this form

Please complete and submit the referral form.  The form will be submitted to Dartmouth Friday Hub and you should also receive an email containing details of the referral request.  Please print the email for your client and give it to them.  Tell them to visit Dartmouth Friday Hub at Townstal Community Hall on any Friday 11.00 am - 12.30 pm to collect their parcel.

Please ensure you give us your work email, showing which agency you represent.  A summary of the referral request details will be sent to this email address.

NB: It is important that you print the email and let them have it.  Without the printed email they may not be able to receive their food parcels.
